All portraits in the game are in one huge image named portraits.dds. It's like a huge version of my “portrait replacement” image, that has place for 32×32 = 1024 portraits, right now only 411 portrait-slots are used. Changing an existing one is just a matter of copy & paste. To add a new portrait (i.e. #412), one needs to find the portrait's number in the game data.
Obviously modifying or adding more portraits will require some coordination effort. To make your mod compatible with mine, you need to include my portraits. (feel free to do so ;)) Adding new portraits will be even more tedious, since there will be additional conflicts regarding which mods use which portrait-slots.
The game only accepts dds files (which is a bad choice for 64 x 64 images IMO). There are DDS import/export filters for GIMP and Photoshop.
nother tip: Don't save the image with MIP maps. Sometimes the game will get confused and used the “zoomed out” version of the MIP map (in other words all the portraits become blurs).
Yes, usually mip maps for 2d images are just a waste of disc space, but here the game seems to handle the portraits just like textures, they seem to scale with the screen resolution. When you enlarge the dds file to 4096 x 4096 the game won't care and simply use 128×128 for every single portrait. This would allow for some nice details once our PCs can run the game in 4k resolution.
when saving as dds, you need to select “No MIP Maps” in the dds-dialog.

Then I moved the two files [PL_F_Head_O_DM.dds] and [PL_F_Head_O_SM.dds] into the [Female armor by Mr.Esturk]'s [Player] folder under […\Characters\Player].

Custom portraits are no longer working after this latest patch.
My custom portrait DDS file is still in place at - Data\Public\Main\Assets\Textures\Icons\
As a test tried starting a new game and scrolling through all portrait options.
You can see the portraits are different so something has changed that now ignores my custom DDS.
How do I get my custom portraits back in the game, this is very important to me.
EDIT: Ok, so I unpacked the Textures.pak file and it seems now the game has divided the big 'portraits.dds' file into smaller more specific ones. The characters portraits are now in a file called 'Portraits_MainPL_CP_Icons.dds'. I've never unpacked this file before, so maybe it was already this way before the patch? Someone correct me if I am wrong, please.
EDIT2: Confirmed. Editing the Portraits_MainPL_CP_Icons.dds file and putting it where you previously would put the portraits.dds file (Data\Public\Main\Assets\Texture\Icons) does correctly change the main characters' portraits in game.
You could use an .pak extractor (https://dl.dropboxusercontent.com/u/32263228/lstools.zip) and extract the Textures.pak, inside the 'Divinity - Original Sin/Data' folder and then search for the “Portraits_MainPL_CP_Icons.dds” file yourself, and then edit it.

You can then edit it yourself (There are plenty of ways to do it, you could use a converter to .png or a software capable of opening and editing the file. https://www.google.com/?gfe_rd=cr&ei=Ic0YVOmwC6Ok8we6QQ&gws_rd=cr#safe=off&q=.dds+editor).
Then just put the edited file in the 'Divinity - Original Sin\Data\Public\Main\Assets\Textures\Icons' folder. If you don't have that folder already, just create it.
That file from nexus is outdated, meaning it's still the old portrait file where all the portraits were together in a single huge file. With the update they divided the portraits into various more specific files. If you downloaded my file, you can edit it and it will work.
If you're using a software to edit it, instead of converting it, remember, when saving, to NOT generate MIP maps. Usualy the software give you this option when saving it.
